• Applicable to Hong Kong residents and Mainland Residents.
  • All persons holding valid identity documents can submit eHealth application online.
  • The following conditions are not applicable for online registration:
    • Newborn without the Hong Kong Birth Certificate
    • Applications by the child's guardian, person appointed by the court to manage his / her affairs, prescribed healthcare provider (HCP) who provides or is about to provide healthcare to him / her, and the Director of Social Welfare
    • Applications by the guardian of a person incapable of giving consent, person appointed by the court to manage his / her affairs, prescribed HCP who provides or is about to provide healthcare to him / her, and the Director of Social Welfare
  • The following Hong Kong residents can register eHealth online with iAM Smart, complete identity verification and eHealth registration instantly.
    • Aged 16 or above and apply with “iAM Smart”
    • Aged between 11-15, applied by substitute decision maker (SDM) (Applicable to a child’s parent, family member or a person residing with him/her)
    • Aged 16 or above who is incapable of giving own consent, applied by SDM (Applicable to the person’s family member or a person residing with him/her)
  • Mainland Residents can click here for details of other registration means. If applicants have submitted application online (not using "iAM Smart"), fax, postal mail or drop-in box (i.e. written means)*, they will receive a notification through email or SMS to complete identity verification for themselves/ their child / their family members / the person incapable of giving consent. Applicants may then bring their identity document and a copy of the person’s identity document (applicable to application by a substitute decision maker (SDM)) when their next visit a registered private healthcare provider providing healthcare to them, or the following locations, for identity verification:
    • Electronic Health Record (eHR) Registration Centres (RC) in the Hospital Authority (HA) or the Department of Health (DH)
    • eHR Registration Office (RO)
    • mobile registration counters
  • If applicants have submitted application as a SDM and are the person's guardian, person appointed by the court to manage his/ her affairs, or prescribed healthcare provider who provides or is about to provide healthcare to him/ her, please also bring other required documents.

* Note:
If applicants have submitted your application online or via written means and are unable to verify their identity in person at the above locations, applicants can authorise a person to complete identity verification on their behalf by specifying on the online submission form or authorisation letter accordingly. The authorised person should bring his / her identity document and a copy of applicants’ identity documents for identity verification.

  • Except the Hospital Authority (HA) and the Department of Health (DH)*, participating healthcare providers (HCPs) can only view and upload patients’ medical records after obtaining patients’ sharing consent.
  • Patients can give sharing consent to up to three participating HCPs (except HA and DH)* when registering with eHealth online.

* Managing sharing consent is not applicable to HA and DH. After joining eHealth as a participating patient, patient are taken to have given sharing consent to HA and DH.

  • Applicants will receive a notification of successful registration with an 8-digit access key via the selected communication mean (such as email).
  • Under normal circumstances, electronic health record within the eHealth sharable scope would be shared to the system around 30 minutes to 1 hour after identity verification.
