  • Father, mother, or legal guardian of babies and preschool children who are the current clients of Maternal and Child Health Centre (MCHC) Child Health Service (including Hong Kong and Mainland residents)
  • An application for replacement should only be made after the original Immunisation Card (DH6) has been lost. In case the original card is found after a replacement has been issued, the applicant should bring both the original and the replacement cards back to the MCHC for further management.
  • Applicant can submit online application via GovHK (Digital signature by “iAM Smart” or e-Cert (personal) is required)
  • Applicant can also download the application form and submit the completed application form and the required documents by mail or in person to the Maternal and Child Health Centre concerned.
  • Need to produce the following documents:
    • Applicant’s HKID card (either original or photocopy)
    • The child’s birth certificate (either original or copy)
    • Documentary evidence showing applicant’s relationship with the child if the applicant is the legal guardian or a parent of the child whose name is not shown on the child’s birth certificate
  • Free of charge
  • Upon receiving the notification, the applicant must collect it within 3 months by one of the following methods:
    • By registered post to the Hong Kong postage address provided by the applicant; or.
    • In person or by an authorised representative Note at Maternal and Child Health Centre concerned

Note: The authorised representative must present the Authorisation for collection of copy of health record / medical report (FHS 123) and proof of the individual’s identity e.g. Hong Kong identity card.

Application for Copy of Childhood Immunisation Record
  • Clients who have received childhood immunisation in MCHC, Student Health Service Centre and/or by School Immunisation Teams (SIT)
  • Child under 18: The application must be submitted by the child’s father, mother, or legal guardian
 Apply for a copy of Childhood Immunisation Record instead of replacement
  • Only accepts application from data subjects under 25 and who have previously received vaccinations from the Department of Health (DH). Among these records, MCHCs and/or SIT only keep the record until the data subject reaches 21 years old. For those aged 21 to under 25, only Student Health Service records will be provided (if applicable)
