• Clients who have received childhood immunisation in Maternal and Child Health Centre (MCHC), Student Health Service Centre and/or by School Immunisation Teams (SIT) (including Hong Kong and Mainland residents)
  • Child under 18 years old: The application must be submitted by the child’s father, mother, or legal guardian
  • Persons aged above 18 and under 25: The application can be submitted by the person themselves. Among these records, MCHCs and/or SIT only keep the record until the client reaches 21 years old. For those aged 21 to under 25, only Student Health Service records will be provided (if applicable)
  • Persons aged 25 or above: No information will be provided
  • Applicant can submit online application via GovHK (Digital signature by “iAM Smart” or e-Cert (personal) is required)
  • Applicant can also download the application form and submit the completed form to dh_vac_copy@dh.gov.hk
  • Submit the completed form in person or post to
    • Maternal and Child Health Centres
    • School Immunisation Teams Tsuen Wan Office
    • Student Health Service Centres
  • Need to produce the following documents:
    • Birth certificate and HKID card* (if available) of the data subject (either original or photocopy)
    • HKID card* of the applicant (either original or photocopy)
    • Documentary evidence showing the relationship between the applicant and the data subject if the applicant’s name is not shown on the birth certificate
    *Valid travel document if HKID is not available
  • HK$160 (After receiving notification from the Department of Health)
  • Payment methods
    • General Demand Note: The Department of Health will send the General Demand Note by email to applicants who choose to receive the copy of Childhood Immunisation Record by email. Please click here to refer to the payment methods for the General Demand Note; or
    • Cash or Octopus or Faster Payment System (FPS): Applicable to payment in MCHCs or Student Health Service Centres
  • The application result will be notified via email or phone within 30 working days upon receiving the application form.
  • Upon receiving the notification, the applicant must collect it within 3 months by one of the following methods:
    • Receive an electronic Childhood Immunisation Record by email
    • Collect a copy of Childhood Immunisation Record at the designated centre in person or by an authorised representative Note

Note: The authorised representative must present the Authorisation for collection of copy of health record / medical report (FHS 123) and proof of the individual’s identity e.g. Hong Kong identity card.
